The Apache County Live Free Reentry Project provides supportive services, information referrals and mentor pairing for individuals re-entering our community from jail/incarceration. Providing these supportive services gives these individuals hope and guidance for a more productive and successful future, empowering them to overcome potential barriers. The volunteer-based, Community Coach Mentorship Program is at the heart of Live Free Reentry.

RE-ENTRY PROGRAMS
WARM HAND-OFFProvides a warm handoff to community-based services as well as connections to supports such as housing and food.

COMMUNITY COACHINGProvides mentoring services to help individuals overcome obstacles associated with returning to the community.

INDEPENDENT REFERRALSProvides referrals to community services and supports such as healthcare, dental care, SUD treatment, and public assistance.
